Registration and consent forms are required prior to your first appointment at Southern Women's Health (SWH).

You will receive the registration form by SMS once you have made your appointment. You can also access the registration form here. Please complete this form prior to your first appointment.

If you would like medical information to be sent to SWH from another service you have visited, please complete this authority to release information consent form and give it to your other health centre to provide these details. If you have seen Dr Danielle at Leichhardt Women's Community Health Centre, you will also need to complete these forms to get your medical history from your attendance there, to SWH.
